Forms
Confidentiality Agreement
Office of the Vice President for Research and Innovation (OVPRI) employees, volunteers, visitors, student workers, and research associates, or those under contract with OVPRI, may have access to confidential employee and financial information. Members of these groups are required to sign a confidentiality agreement.
OVPRI Data Request Form
Submit this form to request access to data maintained by the Office of the Vice President for Research and Innovation (OVPRI).
Cognos Access Form
OVPRI employees should submit this form to request access to the VPRI folders in the Integrated Data and Reporting (Cognos) data warehouse.
