Request for Permission to Transport Animals Using a Private or Non-Fleet Vehicle

In addition to this completed form, UO IACUC policy requires inspection of the vehicle. Animal Welfare Services will contact you to arrange the initial inspection.

For the standards governing transport of animals on campus please see the IACUC Policy on Transportation of Animals. The University of Oregon cannot be responsible for the effect of such transport on a private vehicle or any damage that may occur during transport. Any damage to animals would potentially be covered.

Under no circumstances may animals be transported using public transportation, for example, any bus or shuttle operated by the campus or the city.
